TNPCS work collectively with your team of Allied Health workers, Neuro Psychologists and Support Co-Ordinators to create a team that is best suited to your needs.

In Home Support

The Neuro Practice Carer Support provides assistance with day to day tasks and activities, so you can remain living at home. 

Providing quality support begins with planning and assessment.

Before starting new services, we complete a support plan with you to make sure that:

  • We understand and discuss your preferences and choices about what, how and when services are provided to best suit you

  • We are aware of any specific health conditions that may impact on your support and your team members providing support to you

  • Your home and other environments where our team members support you are safe for both you and the team members.

 Our individualised support plans are reviewed and evaluated at regular intervals.

 Personal Support services include a range of activities such as:

  • Hygiene, grooming and dressing

  • Support to complete therapy and exercise programs including hydrotherapy

  • Meal preparation and assistance with eating and drinking

  • Continence management 

  • Help to utilise aids and equipment such as hoists and wheelchairs

  • Community outings and appointments, leisure and vocational activities.

Community Inclusion

The Neuro Practice Carer Support has a focus on ensuring its participants have every opportunity to be active and valuable members of their community.

Community inclusion for participants has included;

  • Accompanied 1 : 1 trips interstate

  • Animal therapy

  • Transport/assistance to see events, shows and exhibitions

  • Travel to AFL games

  • After school activities 

  • Support to attend educational seminars and groups

  • Opportunities to attend social gatherings 

  • Assistance to attend appointments and therapy programs

Therapy Support

We are able to assist you with your therapy both at home and in the community to promote improved independence including:

  • Home exercise programs to increase strength, balance and mobility

  • Hydrotherapy

  • Gym/strengthening programs tailored to your level of physical ability

  • Assistance with activities of daily living including meal preparation, personal care, weekly planning and domestic tasks

  • Help with use of communication aids and designed speech therapy exercises

  • Attendance at peer and social groups to help you feel confident as part of the community

  • Shopping and recreational activities
